In the event of an accident or illness, I agree to the following:
1) I give permission for my child to receive appropriate first aid treatment by the Pine Lake Christian Camp nurse or trained staff. 2) In the event that I cannot be contacted and emergency medical treatment is needed, I give permission for a licensed physician to hospitalize and secure proper treatment, including injections, anesthesia or surgery for my child. 3) While the staff will make every reasonable effort to teach my child the proper outdoor techniques and minimize exposure to known risks, I acknowledge that all dangers associated with camp activities cannot be forseen.
I hereby release and discharge the Iowa Baptist Conference and Pine Lake Christian Camp, its officers, directors, employees and agents from any claims, causes of action, costs, obligations or financial responsibility resulting from or arising out of any accident occurring while my child is participating in activities at Pine Lake Christian Camp. If the Iowa baptist Conference or Pine Lake Christian Camp is held financially responsible to or for my child for any such incident, injury or accident, I agree to indemnify and hold harmless the Iowa Baptist Conference and Pine Lake Christian Camp from any such responsibility including costs, damages and attorney fees incurred by the Iowa Baptist Conference and Pine Lake Christian Camp.
I hereby give permission for the Iowa Baptist Conference and Pine Lake
Christian Camp to use comments, photographs and videos of my child to
be used in camp promotions. I also give authority in matters of
discipline, understanding that any camper disregarding camp rules is
subject to being sent home at camper expense, with no refund of camper
fees, and that any camper willfully destroying property will be charged
accordingly.
