	Life Group Room Reservation Form Please fill out the form below to reserve a room for your Life Group usage. The form must be turned in a minimum of two weeks before your scheduled event. You will be contacted within one business week of submission. Please notify the church immediately if you are not going to be meeting. You may contact Connie Sieferman at csieferman@kingswaychurch.org. Please notify even if it is the day of the meeting. This is very important. There is always a possibility that your reservation may be cancelled or moved due to larger events taking place in the church or school. Workers who have never worked in the rooms must be trained by Mike Johnson. You will need to reserve the Recreation Center, if available, for children over age 8. Please be aware that you may share the Recreation Center if another Life Group is using it.

	Childcare Form Please fill out this section if you will have children present at your meeting time. Rooms will be assigned on a first-come first-serve basis.
	I/we understand the maximum child/adult ratio will be 10/1. If the majority of the children are preschool age, the ratio is 7/1. If you have nursery aged children, the ratio is 5/1. Minimum age for childcare workers will be 16. I/we understand there should always be one worker in a room over the age of 21 and a minimum of 2 workers in a room
	I/we understand resources (toys, games, etc.) should be obtained from the Church Resource Room (B112) for church-related events or brought on your own. Any resources which are stored in the room should not be used under any circumstances. Please take home any resources you bring with you. You may use toys which are already out.
	Children that have had a fever within the last 24 hours, have colored discharge from their nose, pink eye, diarrhea, or excessive coughing will not be present in this room.
	I/we understand groups reserving the room are responsible for putting all used items away. Rooms should be picked up. I/we understand tables and chairs should be wiped down after use. Rooms do not have to be swept or trash taken out, with the exception of used diapers. I/we understand that soiled diapers be removed from the room as you leave and put in trash bin outside of the Family Life Kitchen. As a general rule, LEAVE THE ROOM AS YOU FOUND IT, OR CLEANER.
	I/we understand that food is allowed in these rooms. You must take leftover food with you as you leave the room. Please try to use pre-packaged snacks. Clear colored liquids only for drinks. The facility use guidelines specifies that red and purple colored drinks are not permitted anywhere in the building.
	I/We understand toys that are out in the room may be used. Toys stored in cabinets or storage closets should not be used. You may bring toys in on your own and they should be taken home when you leave. Toys that are used from the room should be cleaned.
