Before beginning this form, please take a moment to review the following important details: 

Application deadline: For funding eligibility, your application must be submitted by February 15, 2010. Applications will be reviewed promptly and awards notifications sent out by e-mail on February 22, 2010.

Eligible Training Expenses: Companies can select their own training vendor and location. The training course must be open (at least 10% of available seats) to all companies (if the companies can attend). The training course(s) must be completed by June 1, 2010.

The following training programs are not allowed:

• Industry Conferences
• ESL (English as Second Language)
• Basic training covered by WEDNET funds
• Proprietary training owned by a single business
• Basic occupational health & safety training

All training courses must be approved by the Industry Partnership Workforce Development Committee and the employer will be reimbursed for 75% of the training cost. The companies must also be willing to submit the social security numbers of each trainee (on a separate, secured form to be provided later).

The total amount of the requested courses should not exceed $10,000.00.
