	1. I am at least twenty one (21) years of age and a legal U.S. citizen or permanent legal U.S. resident. 2. I have disclosed, below, to the best of my knowledge, anyone I know who is now, or has been in the past two (2) years, an officer, director, employee, agent or representative of: (a) Upper Ground Enterprises, Inc., A.
